The Patient Communication Tool consists of two forms, the Patient Assessment and the Patient Dashboard, and a Color Interpretation Chart explaining what the colors green, yellow, and red represent.

The Patient Communication Tool – Assessment

Used during one of the first conversations between the medical professional and the patient and/or patient’s caregivers, the Patient Assessment visually aids the medical professional while discussing the patient’s overall status.

The medical professional is able to present the patient’s underlying conditions such as age, blood pressure, cancer, chronic conditions, diabetes, weight, etc., as either encouraging or concerning to the patient’s overall health.

Based on the patient’s medical information and professional judgement, the medical professional color codes each organ on the patient graphic as either green, yellow or red.

Basic meanings are:

  • Green represents an organ that is functioning.
  • Yellow represents an organ that is slowing or no longer functioning normally.
  • Red represents an organ that has failed.

The medical professional is also able to add additional notes regarding specific areas of concern to the patient and/or their caregivers.

The Patient Assessment becomes the foundation for subsequent conversations regarding the patient’s status.

Using the Patient Dashboard, the medical professional updates the patient’s status during each subsequent conversations helping the patient and/or their caregiver see which areas are improving, staying the same, or declining.

During the conversation, the medical professional can provide supporting medical information regarding the patient’s status at the level of detail desired and understood by the patient and/or caregivers.

Designed to stay in the patient’s room and be readily available to the patient at any time, the Patient Communication Tool continues to be a visual anchor during subsequent conversations between patients and their loved ones.

The Color Interpretation Chart explains the meaning of the three colors, green, yellow and red, used on the patient graphic shown on the Patient Assessment and Patient Dashboard forms.
